Termination Notice

Use this form to formally notify DJTC of your intention to end the Centre Partnership agreement. Please read the terms below before submitting.

⚠️ Before You Submit

A minimum of 1 week notice is required before termination takes effect.
During this week, both parties must remove references to the partnership from all online materials.
All outstanding settlements owed to you will be paid within 7 days of the effective date.
You must immediately stop using the DJTC Verified Partner badge and branding upon termination.
All DJTC client data in your possession must be deleted within 14 days.
